Build a student survey form as shown in the diagram.

• For the class you need use a pull-down menu displaying the classes PL1001A, PL1001B PL1001C, and PL1001D. Make your class as the default.

• For the Module you need use a pull-down menu displaying Internet Fundamental and Applications, and Engineering Essentials. Make Internet Fundamental and Applications as the default.

• Student’s name is a required field, thus you need to write a javascript to validate it.
• Use either embedded or external CSS to set the background colour to Yellow and the H1 colour to blue (for Student Survey Form).

• Clicking on Terms & Conditions link will open the TC.HTML page shown below in a new window.

· The TC.HTML has three frames as shown above,

where background colour top and bottom frames is set to SILVER and the middle frame is set to LIME.

· The College East logo links to College East home page.

· Please note that the font used is "AR BLANCA" and the font size is set at 5 for the terms and conditions list.
